Charge or waive setup fees when creating subscriptions

If your plan carries a setup fee, you now decide per subscription whether to charge or waive it. A new checkbox at the end of the Create Subscription form defaults to the same logic used at signup — with help text explaining why — and you can override it either way. The summary preview reflects your choice.
This also fixes a quiet gap: bill-later subscriptions used to skip the setup fee entirely. Now the fee is remembered and added to the first invoice at cutoff, charged once, and never re-charged on renewals. One-time-only fees are now waived based on a prior subscription to the same plan, so add-on plans charge their own fees independently — and the invoice preview shows the pending fee before cutoff.
